Should this be double nutted at the top? or should there be a nut on either side of the casing?

I'm pretty sure that's right, I put it that way and make sure that the top nut locks the bottom nut in place... Never had an issue doing it that way, and I believe that the last bike I rode also had it that way... Can't 100% guarantee it's right, but I'd say about 95% sure...
